Sign a health undertaking
A Medical Officer of the Commonwealth (MOC) has determined that you need to complete and sign a Health Undertaking. A Health Undertaking is frequently required for visa applicants in a range of circumstances including evidence of exposure to Tuberculosis (TB) or other health conditions of concern.
You must complete and sign Form 815 Health Undertaking, make a copy for your records, and return it to this office within the timeframe specified in the covering letter. Form 815 Health Undertaking is available on our website immi.homeaffairs.gov.au/help-support/ departmental-forms/pdf-forms
By signing this form you agree to contact the Health Undertaking Service (HUS) which is managed by Bupa Medical Visa Services on behalf of the Department of Home Affairs, on 1300 794 919 during business hours or by email at enquiries@bupamvs.com.au within four weeks of your arrival in Australia or as directed by your case officer, if you are granted the visa. It also means that where directed, you agree to attend any required follow-up medical assessment.
You must include on the form an Australian address where you may be contacted after arriving in Australia. You should provide the HUS with the HAP ID associated with your immigration health examinations where possible. Your HAP ID will have been included in a Health Examination List letter or other equivalent documentation provided to you by the Department.
When your signed Health Undertaking has been received by the Department, your application will continue to be processed and you will be notified once a decision is made.

Keep a copy of the signed form with you as you will need it for reference if you are granted the visa and travel to Australia. |
