请教一道community service的题。有分相谢

原题：When a case is being transferred to another agency, what documentation needs to be completed by the receiving agency? What documentation needs to be transferred to the receiving agency? How quickly should this be done? Discuss in 100–120 words and list at least five types of document or records that might be included.

我在学习资料上没找到，用了各种问法，在网上也没有查到相关的文件和记录到底是哪些。一共要求有五种。
想了半天：应该有personal background information。 medical records。client particular needs documents
referral service organisation recommend records。还应该有一些其他的文件么？尤其是最后一问：how quickly should this be done? 找不到正确答案不敢往上写。交上去fail了就惨了。

我以前就是在community service做的，但是不是case officer
这个可能要看是哪个业务吧，out of home care吗？每个业务不一样吧
