Hi.
Prolonged sitting significantly increases the pressure on the discs in your back and can potentially cause problems.
If you have had constant pain for 3 years it's possible to develop secondary problems in the surrounding tissues - muscle weakness, joint stiffness, nerve tension etc.
I'm not able to treat unless I see you face-to-face, so I'd strongly advise you see your local physiotherapist, take your Xray / CT reports alone with you. They will be able to assess for the above things and give you treatment accordingly.
Here are some general advise I'd give:
1. avoid prolonged sitting, if 15min is your limit, get up regularly and walk around / stretch.
2. Sit with good posture - 90 deg angles at hips / knees / elbows when you are at your desk. Sit as closely to the desk as you can to avoid forward slump. Use a rolled-up towel behind your lumbar spine for support when you sit. This goes for driving too if you need to spend a lot of time in the car. Make sure you maintain normal curve in your lumbar spine.
3. Watch out for your back when you lift - bend you knees instead of your back, avoid twisting.
