向大家问一下MRI的东西.

本人一直腰痛难忍.近日在gp的推荐下想去做个MRI.我找了两个gp开的单.
一个医生(这个医生不能说国语,只能说白话)开的原文是:MRI-SPINE-LUMBAR SACRAL SPINE 备注是:low lumbar back pain getting worse,

另一个医生(这个医生国语,白话都很流利)开的原文是:MRI-SPINE-LUMBAR  备注是:把我描述的病情全部都写进去了,写了不少.比较腿麻,弯腰微痛之类的.

这两个开的MRI检查的区域是不是不同?我该用哪一个.我也不太好意思质问医生为什么不同(因为我跟他们都是说的只看这一个,不看其它医生)?

做一次MRI不便宜.在这里问下大家.该信哪一个?

i dont think there will be much difference, those two regions are right next to each other but the first one may cover more area.   i guess it depends on if ur doc is suspecting any problems from the sacral area / sacroiliac jpint etc.   i suppose u could always ask the radiographer how low the scan will actually go. .

我今天检查回来了,是肌肉的问题,肌肉有些部分是黑的,用医生的话说就是没有水分了,紧挨着骨头,可以想象多痛了.用中医的话说是腰肌劳损.医生推荐我去做物理治疗或是找中医弄弄.这儿找个像样的物理治疗不容易.你说我该怎么办.

我做的免费的， 而且明天开始做物理治疗， 也是免费的。

每年有5次还是10次的免费物理治疗，medicare cover的，不知道您用了没

medicare还cover physio？不都是私保才cover么？而且就算是私保一般的plan一年也就就够cover个三到四次。

There is a scheme callled "Enhanced Primary Care" or "Chronic disease management" that allows for 5 sessions a year with a private physio.

Just note that 5 sessions covers physio, OT, chiro, dietetics, podiatry etc in total.  This means if you have spend 2 sessions on physio you can only have 3 sessions on something else.  

The GP has to fill out the plan and referral form for you to use it.  Some people may be required to pay a gap fee if the physio charges more than what medicare covers, it is different between each clinic so just ring them and ask.

Also because the back pain is chronic and has been going on for years, it's not likely to completely resolve with just 5 sessions, what you can do is ask your doctor to refer you to a public service (if you can't afford private) at the meantime and just use those 5 session as a gap fill in between.  Just remember some public service may also charge a small fee.

请教一下，这个理疗，是在一般GP那里看病开单子，去其他诊所做吗？这个免费理疗还真是头次听说，脖子不舒服，肩膀不舒服都可以吗？

查了下，这个的前提是有慢性病。Medicare报销每次有限额吗？physio要是收更高是不是自己要出gap？

Eligible Patients
Community-based patients may be eligible if they have a chronic (or terminal) medical condition and their GP has provided the following Chronic Disease Management (CDM) services:

    A GP Management Plan (GPMP) - (item 721) and
    Team Care Arrangements (TCAs) - (item 723).

Residents of a residential aged care facility may be eligible if their GP has contributed to a multidisciplinary care plan prepared for them by the residential aged care facility or to a review of the multidisciplinary care plan (item 731).

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions. However, the CDM items are designed for patients who require a structured approach and to enable GPs to plan and coordinate the care of patients with complex conditions requiring ongoing care from a multidisciplinary team.

Patients have complex care needs if they need ongoing care from a multidisciplinary team consisting of their GP and at least two other health or care providers.

gp那开单子，华人gp一般都会给开吧。。。那5次 还包括其它。。。比如理疗，podiatry，occupational therapy, speech therapy. ..

昨天被告知这个要看专科然后才给5次免费 ，看专科得掏钱，当然报一部分的。
先前一个医生说想要5次免费，必须要5个专业人士给建立一个护理计划。
我是肩部扭伤。

According to the Department of Health you don't need to see the specialist for that.  Whether you are eligible is the GP's judgement.  

And you don't need 5 different professionals to treat a shoulder strain, where do they expect you to find 5 different professionals to write up 1 plan for a shoulder strain?  

I have attached what's on the Department of Health website.  


"The Chronic Disease Management (formerly Enhanced Primary Care or EPC) — GP services on the Medicare Benefits Schedule (MBS) enable GPs to plan and coordinate the health care of patients with chronic or terminal medical conditions, including patients with these conditions who require multidisciplinary, team-based care from a GP and at least two other health or care providers.

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions; however, the CDM items are designed for patients who require a structured approach, including those requiring ongoing care from a multidisciplinary team.

今天跟我们单位的GP聊了一下，她说要有三个医务人员同时看这个才能qualify，不用五个哦。医生本人算一个。你同时再找一个理疗师什么的，然后如果疼痛严重影响到精神健康你还可以再找一个心理医生什么的，反正凑齐三个。我问她那如果头一次和医生谈起这个毛病是不是不能用，她说你可以先去付钱看一次，然后就能凑三个。-      -

I hope you get through quicker, but do remember it has to be a chronic condition going for more than 6months.  If it's an acute injury the GP may say it's not eligible.

感谢你耐心详细的解答，我去看了两个医生，两个医生给出不同的解决方案，或许我应该看第三个医生，根据你提供的资料我的情况是符合的。

请问这五次免费是指MEDICARE  给全部报销吗。只要有专科大夫的推荐
