麻醉师的费用是怎么收的？

刚收到生孩子麻醉师的账单，没有full bill只发了需要付500gap。
正好收到medicare和bupa的claim单子，发现数对不上：

medicare上显示这个麻醉师收费一共$1370，medicare报了400左右。
bupa上也是显示一共收费$1370，bupa报了800多。那我可不可以理解为gap只剩下不到200来块钱？？
那为啥我还要付500gap呢？

然后给他们发了邮件要求要个full invoice，没收到邮件回复，但是回复了信息：
Dear xxx as you can see above you were advised in writing that an out of poket fee 500 would be charged. all patients attending xxx hospital sigh to acknowledge that there will be out of pocket fees for varying services, the account sent to you is an out of pocket gap account.
（因为进医院前一天前台发了信息告知医院分了xxx医生，我需要付500gap。）

现在纠结要继续问个究竟还是付掉算了。。。我付500gap没关系，但是是不是说明他们给medicare和bupa的claim数额是不对的？
还是说这些私立医院的麻醉师就是这样操作的？？

我老婆之前一个小手术的钱150刀左右就是直接转账给那个麻醉师的-_-

很多时候麻醉师都是分开收费计算的，可能是药物的费用？

Sorry, I type English is faster than Chinese
Basically the gap they charge is not counted according the invoice they gave to Medicare N insurance company. Because they set codes and fees ranges for anaethetist for every type of anesthesia. But even the same surgery operates on different patients, the requirements are different. For example, same D&C surgery, operating on healthy patient, only take 2 Minutes  for anaethetist
Put in airway, and take surgeon 5 mins to finish surgery, the out of pocket will be cheaper, Compare to over weight pt with heart problems, may take anaethetist half an hour to put airway plus invasive monitor in, of course the out of pocket will be much higher on this patient even though undergo same surgery. Really depends the length of surgery,how difficult is the intubation, patient s history, any blocks done on patient....., You can't aurgue, can only ask nicely whether can reduce a bit of charges, before covid , simple D&C will be 250-300$out of pocket, yours doesn't sound too expensive to me.

Just saw the fees was for you given birth, for example, if you require anaethetist during business hours, he will charge you lesser than you call for his help at middle of night, but  Medicare N insurance company only pay him a fixed fee for the same surgery, that's why they are entitled for charging  gap.
