1) The two are acceptable. However, the 1st one is the best of all.
Subject | adverb + verb | object | I | only know | him. | We | hardly/barely knew | the family. |
2) Generally speaking, I won't use Negative Infinitive in any formal writings - when writing poems or drawing a comparison, I may do, so I'd say '... didn't happen to...'.
In terms of spoken English, some verbs like 'decide' go with 'Not To' perfectly well e.g.
- Initially I thought ..., then I decided not to go.
- She told me that ... and she asked me not to tell her husband.
- Interestingly A birds happened to evolve; B birds happened not to ...
