急！帮忙看儿童消炎药水的用量

不知道有没有做医生或者药房的朋友？或者家里孩子吃这种药水的父母们？

我宝宝6岁，21kg。肺部感染发烧，之前的gp几次都是配AUGMENTIN Duo400(400 mg AMOXYCILLIN and 57 mg POTASSIUM CLAVULANATE)，一天2次每次2.8ml。今天换了个gp，说按照我宝宝体重不应该吃那么少，至少应该是5-6ml一次，每天2次。我说之前医生都是这样配药的，曾经在2、3个药房配药，药房的人都没有说我的剂量太小啊。这个gp说药房不会管剂量低的，只有太高了他们才会告诉顾客。

然后我就按照一天2次每次5ml去药房配药。这个药房的人正好是ABC，能说一点中文，他说帮我算一算。算好后他说这个gp是对的，应该5ml。然后他说药房应该都要帮客户检查剂量的，剂量低也应该告诉你的。

所以我想请论坛上有没有懂药的朋友，或者能否帮忙看看我宝宝到底应该吃多少？

我拍个药水的照片上来，如果家里有类似体重宝宝吃过这药水的爸爸妈妈们，能告诉我你们的孩子吃几毫升吗？（这次gp和药房的人说，这个药水的浓度duo400是固定的，每家药房配的浓度都是一样的）

如果是家里孩子吃的话，一定要同样是这种药水的，我才能参考呀。
阿莫西林药水我们是吃5ml的。

所以建议再问医生，药真是不能随便吃的

根据药水浓度，才能算出该吃多少

5mls twice a day

药房和这次的gp都说只要是这个药，浓度都是固定的，不存在哪个gp开的浓度高少吃点哪个gp开的浓度低多吃点的情况。
我拍照是为了让不懂医的妈妈（家里孩子吃过这个药的）来说说看自己孩子多少体重，吃多少ml。

一般孩子的阿莫西林剂量是12.5mg/kg， 所以12.5 x 21kg = 262.5mg.
你的药水是400mg/5mL, 所以剂量应该是3-3.5mL一天三次，或者5mL一天两次。

amoxycillin和amoxycillin with clavulanic acid的剂量是不一样的

我刚刚搜了网上，Amoxicillin and Clavulanate Potassium【其他名称】 奥格门汀，AUGMENTIN
40kg以下儿童剂量：按阿莫西林计算，一般感染每12小时25mg/kg或每8小时20mg/kg。这个算法我算了一下，宝宝应该是每12小时6.56ml。这个和新gp的说法差不多。

英文的：
DOSAGE AND ADMINISTRATION
AUGMENTIN DUO 400 should be taken immediately before or with the first mouthful of food, to
minimise potential gastrointestinal intolerance and to optimise absorption.
Children aged 2 months up to 12 years
For moderate to severe infections the dose should be 45 mg/kg/day, based on the amoxycillin
component, (or 6.4mg/kg/day clavulanic acid) in two divided doses every 12 hours.
The children's dosage is intended for individuals whose weight will not cause dosage to be
calculated greater than that recommended for adults. Children weighing 40 kg and more should be
dosed according to the adult recommendations for other AUGMENTIN preparations.
There are no clinical data available for AUGMENTIN DUO 400 in infants with immature renal
function. The use of AUGMENTIN DUO 400 in this group cannot be recommended.

看了2个英文版本的，差不多都是
25 mg/3.6 mg/kg/day（最小剂量） to 45 mg/6.4 mg/kg/day （最大剂量）given as two divided doses;
我感觉，如果是按最小剂量来算：每天的量25mg的阿莫西林*21kg=525mg，即6.56ml，但是这是每天的量，如果分2次给，则每次是3.28ml吗？如果按每天3.6mg的acid*21kg=75.6mg，即6.63ml，但yeshiva每天的量，每次给3.31ml？
这样的话，这次的gp也给多了？

抗生素根据感染部位不同，剂量也不一样。

To produce Augmentin Duo 400 Suspension (per 5 mL: amoxycillin 400 mg, clavulanic acid 57 mg), use 107 mL glass bottle, add 55 mL water, for final volume of reconstituted oral suspension of 60 mL.
Shake oral suspension well before using. Reconstituted syrup must be stored under refrigeration (2-8deg. C) and discarded after 7 days.

好像以前的gp是按最小量给的，这次gp是按最大量给的？
