最近申請了張amex Q聯名卡
銀行方面寄來要補充資料內容如下

Thank you for applying for A Qantas American Express Discovery Card. At this time, your application is still under review. In order for us to finish processing your application, we need to verify some information in accordance with the Anti-Money Laundering and Counter Terrorism Financing Act 2006.

Please provide us with copies of:

One of the following forms of photographic identification for the Primary Applicant:
Primary photo ID document (Australian Driver's License, valid Passport or Australian Proof of Age Card).
Verification of your current address, such as a recent utility bill / bank statement (within the last 3 months), last year's Notice of Tax Assessment from the ATO or a Council rates notice.

Please note that all documents must be certified as a genuine copy of the original document by any of the following certified signatories:
Registered Accountant.
Justice of the Peace.
Practicing Solicitor.

In order to process the application, please ensure the certifier includes the following information:
Full name.
Qualification/title.
Date of certification.
Signature.
Registration number.
Stamp or registered address of certifier.
The phrase, "I certify this to be a true copy of the original document."

Please write the reference number on each page of the documents. Once you have gathered the documents, there are several ways you can send them to us:

Upload them through our Document Upload Site. For access, you will need to provide the reference number above as well as certain personal information.
