请教MEDICARE中的PEI

最近查血，收到账单，准备去claim。在网上medicare的网站上查了下，似乎所有的项目都能由medicare埋单，除了PEI：

Initiation of a patient episode by collection of a specimen for 1 or more  services (other than those services described in items 73922, 73924 or 73926) if the specimen is collected in an approved collection centre. Unless item 73920 or 73929 applies Fee: $6.00 Benefit: 75% = $4.50 85% = $5.10 我的账单上charge是30.10，就是说只能claim回来30.1-5.1=25回来，是这个意思么？ 这个差别是怎么来的呢？是由于私立医院和公立医院的差别么（检查的机构title是pathology）？
