If you had out-of-pocket medical costs more than $2000, you can claim Medical Tax Offset. However, you would have to provide the following documents for your tax accountant:
1. Annual Benefit Payments Statement issued by Medicare from 1st Jul 2011 to 30th June 2012 for whole family.
2. Annual Benefit Payments Statement issued by private health insurance company from 1st Jul 2011 to 30th June 2012 for whole family.
3. Annual tax statement issued by pharmacists for prescription medicine purchased for whole family.
4. All other medical bills which were not gone through above system, such as dental bills etc
